Using Poindexter, APH's data analytics risk engine custom built for population health management, assess NKCH's healthcare costs, identify opportunities and map out an actionable path to provide a cost-effective, self-funded benefits plan based on increased collaboration and shared risk.

  • Stabilize health plan costs
  • Aggregate and provide data insights, stratify health risks of the population
  • Target areas for improvement in preventive medicine screening and medicine compliance
  • Develop wellness program with on-site wellness coordinator: health risk assessment, biometric testing, and individual health coaching 
  • Implement disease management program for chronic conditions 
  • Train medical team in utilization of Poindexter: APH information used daily in health coaching and primary care physician outreach 
  • Monthly team meeting to review performance and develop strategies for improvement
