Taking the final mile with Population Health data findings - Nurse Navigation is one avenue

More and more frequently, our current and prospective employer group clients will ask us what we see as the most effective avenues to support employees and members who have been identified through analytics for outreach and support.

There are many avenues that our clients are taking to get individuals the education and support they need. Nurse Navigation is one of the most effective programs that we’re partnering with our clients to launch and manage. We thought this would be a good topic to cover during National Nurses Week.

Data Analytics Enables Health Plans to Predict When Employees Need Health Services and For How Long

Speaking of wellness programs, if analytics can predict and help employers better design their health plans, couldn’t the technology also be used to help prevent or put off more adverse and expensive health events

Advanced Plan for Health Upgrades Next-Generation Predictive Analytics On Poindexter Population Health Management Platform

Poindexter’s newest release pushes the boundaries of predictive modeling by determining the likelihood of an occurrence of coronary events, neurological events, orthopedic events and chronic kidney disease. In addition, Poindexter now has the ability to predict hospital length of stay and general likelihood of an ER visit for any member of a patient population.

Third-party Administrators (TPAs) and brokers can leverage Poindexter’s new functionality to better identify at-risk individuals in their customers’ insured population. Employers with self-funded health plans can directly benefit, too.

Nurse Care Navigators' Value to the Self-Insured Health Plan

To nurse care navigators, care management is exactly that – managing people with care.

Mentoring clinicians and health plan professionals in learning how to guide and support people, wherever they are on the healthcare continuum, is my opportunity to pass on the secrets of population health management that so many wonderful professionals have shared with me.

Making a difference in people's health, while optimizing resources so that people and health plans get the desired results, is my idea of success.

Informed and Healthy Employees Make for Healthy Benefit Plans

Employers have found that it is not only the catastrophic conditions inherent in the high risk employee cohort, but also individuals who have reached the high risk group as a result of many different health issues (co-morbidities), lacking a personal physician, and inappropriately using the hospital or emergent care services for preventable problems. Plan members who are bouncing from one physician to another, because they may not understand what they need or where to obtain the requisite services, also present opportunities to improve clinical and financial metrics.

April Showers Bring May Flowers

A great many of things happen in May. The blossoming of trees and plants in the Northern Hemisphere signals the much awaited onset of spring. Even the popular rhyme, “April showers bring May flowers” gains new life as we hear and consciously repeat it with the faith that April rains mean life and color to our settings in May. This rhyme originates from a short poem written in 1557 by Thomas Tusser:
